THESE PRICES ARE EFFECTIVE AS OF August 1, 2010

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, ANY FUNERAL ARRANGEMENT YOU SELECT WILL INCLUDE A CHARGE FOR OUR BASIC SERVICES AND OVERHEAD. If legal or others requirements mean you must purchase any items that you did not specifically ask for, we will explain the reason in writing on the STATEMENT OF FUNERAL GOODS & SERVICES SELECTED.

Professional and Staff Services

A. Basic Services of Funeral Director and Staff

Our professional service charge includes, but is not limited to, staff and basic facilities to respond to initial requests for services on a twenty-four (24) hour basis, arrangement conference with responsible parties to determine desired services, coordinating plans with cemetery, crematory and/or other parties involved in the final disposition of the deceased, obtaining preparing, and filing of necessary notices, authorization and consents, certificates and permits, and sheltering of human remains.

B. Embalming

Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

C. Other Preparation of the Body

This fee is the total charge for all *standard service items* “A” through “R” listed on this General Price List. You may elect not to use any items with the exception of item “A“. This total does not include casket or outer burial container selected. Also, not included in this fee is any costs for obituaries in any newspapers or any copies of the death certificates which are presently $6.00 each.

A funeral director on our staff at the time of making funeral arrangements will supply you with envelopes for you to make your check payable directly to the cemetery and clergy.

Direct Cremations

If you want to arrange a direct cremation, you can use an alternate container. Alternate containers encase the body and can be made of materials such as fiberboard or a composition material made with or without an outside covering. The alternate containers used by our funeral home are made of cardboard and fiberboard.

A. Direct Cremation with container provided by the purchaser or use of a vinyl pouch	$ 3190

Immediate Burial

Forwarding Remains to Another Funeral Home

Receiving Remains from Another Funeral Home
